Montefiore Selects Acupera’s Population Health Management Technology to Support Community Care Efforts

Workflow Technology to Enhance Care Management for Patients with Multiple Chronic Conditions

SAN FRANCISCO—January 14, 2015

Acupera, a leading provider of population health management workflow technology, announced today that Montefiore Health System, a premier academic health system and the University Hospital for Albert Einstein College of Medicine, has selected Acupera to support its care teams who serve medically vulnerable and psychosocially complex patients in its NYS Medicaid Health Home program. Montefiore, which is located in New York, has been a pioneer in cultivating population health strategies aimed at improving community health and well-being.

Acupera’s technology will be integrated into Montefiore’s Department of Pediatrics Intensive Case Management Program which serves vulnerable adult and pediatric patients and their families who are enrolled in the Bronx Accountable Healthcare Network (BAHN) Medicaid Health Home Program, of which Montefiore is the lead participant. Acupera’s system will enable care teams to collaborate across the Health Home’s network of hospitals and community organizations to engage and treat patients struggling with both clinical and socio-behavioral issues.

“Acupera will enhance the ability of our field based case management teams to provide holistic medical and psychosocial care management for a range of conditions; and supporting proactive preventive care and linkage services that will help our clients improve their health and potentially decreasing unnecessary emergency room and hospital admissions,” said Marcy Sedlacek, Ph.D, director of the Intensive Case Management Program.

Acupera will work with the BAHN Health Home on data exchange and the sharing of care plans between its providers and community care teams.

“We selected Acupera for its ability to work within our existing Health Home system and create automated and adaptive clinical workflows, ultimately improving how our clinicians deliver care,” said Stephen Rosenthal, vice president of network management at Montefiore. “Having refined our population health program over the years, we require an intelligent solution that provides our care teams with effective, workflow technology in real-time.”

“Montefiore has a strong reputation for effective population health management, with experience in what it takes to be truly effective in this space,” said Dr. Ronald Razmi, chief executive officer of Acupera. “We’re honored to support the Montefiore care teams by improving collaboration and communication, ultimately increasing overall efficacy and efficiency of the health system.”
