Workflow Technology to Enhance Care Management for Patients with Multiple Chronic Conditions

Acupera, a leading provider of population health management workflow technology, announced today that a premier academic health system and the University Hospital for Albert Einstein College of Medicine, has selected Acupera to support its care teams who serve medically vulnerable and psychosocially complex patients in its NYS Medicaid Health Home program. Montefiore, which is located in New York, has been a pioneer in cultivating population health strategies aimed at improving community health and well-being.

Acupera’s technology will be integrated into Montefiore’s Department of Pediatrics Intensive Case Management Program which serves vulnerable adult and pediatric patients and their families who are enrolled in the Bronx Accountable Healthcare Network (BAHN) Medicaid Health Home Program, of which Montefiore is the lead participant. Acupera’s system will enable care teams to collaborate across the Health Home’s network of hospitals and community organizations to engage and treat patients struggling with both clinical and socio-behavioral issues.
