Clinical Care Pathway Authors

Who we are:

Acupera is a San Francisco digital health company with focus solutions that enable the new paradigms of healthcare delivery.

What we do:

We are developing a portfolio of digital health solutions that assist providers in population health management and care coordination. Our solution will enable provider institutions to successfully risk stratify their patient population and manage the clinical and behavioral issues that drive poor medical outcomes. Our cloud-based analytics and workflow engine allow medical centers to maximize their human resources and achieve best possible clinical outcomes. We have successfully implemented our solution in large health systems and are working with over 2 million patient records that includes EHR data, claims, pharmacy data, devices, etc.

Who we are looking for:

We are looking for medical professionals that are interested in participating in our content creation for specific care pathways that tackle management of clinical conditions. These dynamic pathways include identification rules to identify appropriate patients to screen for the specific condition; co-morbidity screening, identification and management; optimal staging, management and monitoring of the clinical condition; lifestyle issues; self-management; and health literacy related to the clinical condition. Candidates who show high promise will be considered for full time positions.

Responsiblities:

The content authors work with Acupera’s medical team to author care pathways for the specified clinical conditions that are prioritized. Acupera medical team will provide templates and training on the most effective and efficient approaches to authoring these care pathways. The pathways are designed to allow for (1) automatic identification of gaps in care when run against patient data that includes EHR, claims, pharmacy, labs, monitoring devices, etc (2) automated generation of care plans based on the patient’s clinical data (3) guide care managers in assessing patient’s health literacy about their conditions and educating them about their condition (4) guide care managers in assessing patient’s understanding of self-management skills and training them in those condition-specific skills (5) Identify missing labs in correct diagnosis or monitoring of the condition.

Acupera has successfully created care pathway for major chronic conditions and behavioral drivers of health outcomes. These pathways are currently in use in major medical centers and performing extremely well in identifying opportunities to improve clinical care and guiding the workflow of care managers. Acupera will work with the potential authors to train in best practices in authoring these care pathways and provide templates to guide the creation of the next set of care pathways.

The current process for creation and testing of care pathways include:

  • Reviewing existing guidelines and review articles for identification of current accepted best practices in screening, diagnosis, and management of the primary condition and associated co-morbidities
  • Author the care pathway using the Acupera logic and format
  • Test for appropriate flow, logic, and inclusion of all appropriate sections

Qualifications/Requirements:

  • MD or DO required. Residency and some clinical practice desirable but not required
  • Experience with medical content creation or management preferred
  • Senior medical students would be considered
  • Would consider nurses with experience in content creation and management
  • Would consider specialists who can commit to creating pathways for multiple conditions in their area of expertise
  • Prefer candidates who can commit to at least 2-3 pathways per month
  • Need candidates with extreme attention to detail, meticulous, creative, comprehensive, and agile

Director of Clinical Informatics

Who we are looking for:

We are looking for an experienced informatics specialist to be an integral part of the team in product development, data integration, data mapping, care pathway development, and all other matters related to clinical informatics.

Responsiblities:

This position serves as our clinical informatics expert to direct activities associated with clinical information acquisition, storage and exchange, and to facilitate applying information to the requirements of the Acupera system. They will lead data integration projects with EHRs, Claims, labs, and other data sources; be the primary source of guidance for the technology team in all clinical informatics issues; lead efforts to set up terminology services, drug databases, data mapping, etc. The position requires an extremely hands-on individual who has intricate knowledge of clinical informatics and can function independently to complete these projects.

They will lead data integration projects with EHRs, Claims, labs, and other data sources; be the primary source of guidance for the technology team in all clinical informatics issues; lead efforts to set up terminology services, drug databases, data mapping, etc. The position requires an extremely hands-on individual who has intricate knowledge of clinical informatics and can function independently to complete these projects.

This role will also be responsible for defining the measures for our clinical analytics, be able to conduct data analysis, then work with the software engineering team to build in the analytics in the system and verify the results.

Some example tasks they may be responsible for:

  • Given inpatient and outpatient data sources from a single or multiple geographically close health care systems, come up with the best logic to reliably match patients
  • Come up with a mapping scheme to map a custom lab coding scheme to the LOINC terminology
  • Analyze a data set from a customer in a SQL database and figure out the logic to infer that certain exams have been performed
  • For different data sets, come up with the logic to determine clinical criteria such as the occurrence of clinical events, e.g. a diabetic foot exam, since many events are not present as structured claims/ICD-9 codes.

Qualifications/Requirements:

  • Masters in medical informatics
  • Doctor of Medicine with few years of clinical experience preferred but not required
  • Ambulatory clinical experience with EHR architecture and data modeling of clinical administrative and practice databases
  • Full familiarity and ability to work with various clinical data standards such as ICD, RXNORM, LOINC, Snomed, UMLS
  • Must be comfortable to independently view and analyze data in HL7 format such as ADT, CCD, CDA, v2, v3 RIM, etc.
  • Must be comfortable with basic queries in SQL
  • Must be comfortable with 837 claim formats and know how to view an 837 form in a human readable format and find different elements

Healthcare Data Scientist

Who we are looking for:

We are looking for an experienced healthcare data scientist to be an integral part of the team in product development and use our vast data assets to create innovative digital health products that enable new models of healthcare delivery.

Responsiblities:

This position serves as data scientist who will work with our medical and technology team in understanding the key healthcare delivery issues that we’re trying to solve and what products will be able to address those issues. Once a solution has been conceived, the data scientist will devise data analytics and modeling plans to support the product development. The models need to be innovative and interactive. Most will need to update real time based on new information from the many databases that the Acupera system receives live information. The different types of models include risk stratification models to identify different cohorts of patients that are suited for different types of interventions and intensity of care; models that support optimal referral management system; models that support matching of the most effective interventions with different types of drivers of poor health outcomes; dynamic models that support patient prioritization for care management, and more.

The candidate needs to have experience and have built models with different forms of healthcare data, especially EHR data. Acupera possesses large EHR data assets and is looking to combine this data with claims, pharmacy, labs, and other forms of data to create innovative models that improve care delivery at various levels.

This role will also be responsible for defining the measures for our clinical analytics, be able to conduct data analysis, then work with the software engineering team to build in the analytics in the system and verify the results.

Some example tasks they may be responsible for:

  • Create dynamic risk models of a population of patients to identify risk cohorts and also individual risk for high cost health outcomes. The models need to distinguish between actionable and non-actionable cohorts and individuals and update dynamically as new data becomes available
  • Create models that can match the most effective interventions to the identified patient risks
  • Create models that find optimal pathways for patient referrals and ensure the most efficient resolution to patient issues

Qualifications/Requirements:

  • Degree from a top institution in relevant fields such as engineering, statistics, actuarial sciences
  • Education and experience in medical informatics highly preferred
  • Experience with doing analytics and building models using health care data, including EHR data, claims, administrative data, pharmacy data, laboratory data, devices, etc
  • Critical thinking skills to glean insights from the analytics process and apply key learnings to improve models
  • Full familiarity and ability to work with various clinical data standards such as ICD, RXNORM, LOINC, Snomed, UMLS
